What FirstReport Online does and doesn't do with New Reporting of
Workers Comp for Arizona Locations

Create and record an incident data record in FROL
- The same process as in the past
- Adds new data points to satisfy the ICA requirements for the new Online employer form
- New ICA data input fields will be color coded in the related categories and the "Auto-Help" for each new field will appear in the blue box near the form top. The description for the new data points will be white text on a dark gray background. The input fields on the right that have the heavy green border are the fields that are required to be completed in order to submit the online form.
- Record/edit OSHA data as before

Generate, on demand, 5 reports
- Draft FROI on the legacy AZ form (the version currently in FROL). All of the legacy data fields will still be available for ongoing incident management.
- Editable PDF of the answers the Employer must enter in the new ICA form. You can cut and paste from this report to complete the online form.
- Editable PDF that can be shared with the Employee to satisfy the ICA reporting. The report will be generated partially complete with information provided by the employer. The employee can then edit/complete the report and submit it to the ICA.
- OSHA 301 incident summary
- Incident Data Export

Transmit the approved FROI to the Carrier
- When approved by the ICA a code is sent to the Employer
- That code is included in the FROL record via Incident Edit, then saved
- That report can be generated and will include the approval code for the carrier to process
- Or! You can transmit the claim to the carrier as before if the carrier is not requiring the ICA approval code to begin coverage. You should get clarification from your carrier as the ICA documentation is unclear.

Updates
- First reports are just that. The FROL users can edit or update the data records, ongoing, for Management reports and OSHA Recordkeeping

Doesn't Do
- FROL doesn't transmit incdent data to AZ ICA for the Employer or Employee.

In the Record data Entre form There is a new field to enter the ICA Approval Code. The field descriptions with the dark gray background are the fields that are available on the new ICA online employer form. The input fields on the right that have the heavy green border are the fields that are required to be completed in order to submit the form to the state. The thin red border still signifies fields that are required by the FROL application.

At the bottom of the Incident Data form page, new "Reports" options appear.
The FROL Employer's Report to assist in your direct submittal to the ICA.
You can copy and paste values to the ICA form.
