OSHA Log Form Help
Overview
Reports can be generated at any time by users with permission to edit and/or view records. The reports are kept only while the user is logged in to a session. When the user logs out or the session "Times out" the reports are discarded. The Organization Select Form determines the range of content included in the report. By including all locations in the Organization Select Form you can create a 300 log or 300A summary rolled up into one report. This is not part of the the OSHA recordkeeping regulation but is a good management report.

Permissions
All users can generate these reports, including "Read Only" standard Users.
OSHA Reports
All of the reports listed above collect multiple incident records. If the incident data is incorrect or records should be included or excluded, go to the incident records, make the corrections and then regenerate these reports.
Report Year
Select the year to include the correct data for your report. Years displayed in red have no records. Remember that the 300A posted in 2007 should include 2006 data. Selecting the current year will include Year to Date data.
Select Report
- OSHA Log - generates the 300 Log in landscape 8.5 X 11 format. PDF file format. 10 records to a page. Quantities totaled by page, no summation total.
- OSHA Summary - generates the 300A Summary in landscape 8.5 X 11 format. PDF file format.
- OSHA Log and Summary - Generates both above in one command
- Dart Calculation Report - generates two reports, summary calculation and a detail report in landscape 8.5 X 11 format. PDF file format. 10 records to a page. Quantities totaled.
- Edit 300A Employment Information - Take you to the 300A Employment Information worksheet to add "Total Hours Worked" and "Average Number of Employees" to complete the 300A Summary required data.
300 Log - Step by Step
The 300 log includes all incident records checked "Recordable" on the incident form. If a record should not be on the log or is missing from the log, this is corrected by editing the incident record. The report is generated in PDF format printable on 8.5" x 11" paper. Multiple pages are produced if the records cannot fit on one page.
Although OSHA requires a log for a specific street address, the application can generate logs for specific departments, locations or roll up to the employer level on one log.

- Select the year from the drop-down list. Years in red do not contain incidents and will produce blank reports

- Select the report file link (s) which is located under the "Last Report Generated" header near the top of the form
- Adobe Acrobat Reader will provide printing options for you.
300A Summary - Step by Step
The 300A includes a summary of all incident records checked "Recordable" on the incident form. This report is printed and posted in view of all employees from February 1 to April 30 each year for the previous year's history. When you run the report be sure you choose the correct year. You post 2006 data in 2007.

300A Summary Worksheet Export - Step by Step
The 300A includes a summary of all incident records checked "Recordable" on the incident form. This report is printed and posted in view of all employees from February 1 to April 30 each year for the previous year's history.
There are two data points needed to correctly complete the 300A. They are "Average number of employees" and "Total hours worked" for that location for the year. The worksheet is where is data is entered. The application provides an export data option for this data. When you access the worksheet a link to the export file is already prepared.
The data is in "CSV" format readable by Excel or other programs.
The application can generate report for specific departments, locations or roll up to the employer level on one file.
