Several types of reports can be generated once an incident record is created. Reports can be generated at any time by users with permission to edit and/or view the incident record. The reports are kept only while the user is logged in to a session. When the user logs out or the session "Times out" the reports are discarded.
FirstReport Online Technical support does not include printing support. We create reports in standard file formats. If you have difficulty printing PDF files contact your systems administrator.

Incident Reports
Report generated at the incident record level only contain information for that incident. Some reports are defined by the State or Federal agency and others are provided by our service for your benefit.
WC - FROI (first report of injury) and OSHA - 301 (incident summary)
- Open an Incident Record
- Scroll to the bottom of the form
- Select Report Type - FROI (WC ) or OSHA 301 (OSHA)
- Select "Report" - found beside the "Report Type"
- The incident form refreshes to the top of the form
- Select the report link highlighted in red under "Last Report Generated" to open
- Or, Right Click on the link and choose "Save Target as" to save the report
- Adobe Acrobat Reader will provide printing options for you
Incident Record Case Notes
- Open an Incident Record
- Select "Notes" button to view the Case notes for that incident
- Select Report Type - Case Notes
- Select "Report" - found on the bottom of the form
- Select criteria on any of the five desired options including "Report Format"
- Select "Next" - found on the bottom of the form
- Select the report link highlighted in red under "Last Report Generated" to open
- Or, Right Click on the link and choose "Save Target as" to save the report
- Adobe Acrobat Reader will provide printing options for you
CPA - accident analysis report (Special permission required for CPA)
- Open an Incident Record
- Scroll to the bottom of the form
- Select "Analyze"
- Select "Report" - found on the bottom of the form
- Select the report link highlighted in red under "Last Report Generated" to open
- Or, Right Click on the link and choose "Save Target as" to save the report
- Adobe Acrobat Reader will provide printing options for you
CPA - Case Notes
- Open an Incident Record
- Scroll to the bottom of the form
- Select "Analyze"
- Select "CPA notes"
- Select "Report" - found on the bottom of the form
- Select the report link highlighted in red under "Last Report Generated" to open
- Or, Right Click on the link and choose "Save Target as" to save the report
- Adobe Acrobat Reader will provide printing options for you
Export Data
- Scroll to the bottom of the form
- Check the "Export Data" box
- Select "Report"
- Select the report link highlighted in red under "Last Report Generated" to open

The file is a delimited text file formatted to match the chosen printed report.
The delimiting character is the pipe (|) character which is located directly above
the back slash (\) character on your keyboard. This file can be imported into many
spreadsheet or database programs such as Microsoft Excel or Access.
Since the data is formatted to match the current printed report, it is subject to
change without notice as the printed report is revised."
