The LS 202 Form
The LS 202 was released Sunday, 7-17-2011. This form will be activated upon request. The customers that have already requested it will find it activated when released. Others will see it greyed out as with Auto Property and GL for non-subscribers of the accord forms. There is no additional fee for access to the LS 202. Other active customers may request this feature by email at support@firstrecords.com.
It is not based on Jurisdictions as are the State FROI’s. The employer may select the LS 202 and when completed can generate a PDF report. The data form, like all FROI’s, also satisfies OSHA recordkeeping requirements. Additionally, the User may choose to create a State specific FROI by selecting that button at the bottom of the LS 202 Data form. This will prefill the common data fields on the State data form and provide the ability for the creation of the state FROI PDF report. About 18 states require a copy of the incident on their FROI form even though the primary recipient is the DOL. The customer should NOT create an incident record through our traditional method of “Incidents, Create, Osha/WC because there will then be duplicate records and you cannot create a LS 202 from the State Data form, only the inverse.
That data form and the LS 202 data form will have the same FROLID # and the state form can only be accessed through the LS 202 data form or by doing an “Incident, Edit, search” to select it.
The Form Assistant should also be used for the “Carrier” and other data points similarly to the State FROI’s.
All the LS 202 information will be included in any incident searches and Summary Reports as any WC incident record would. The State form cannot be transmitted. The LS 202 can be transmitted via our FAX feature, although the DOL has stated that they will not accept direct FAXes without original signatures.
Important notes on how the LS202 function works:
- Access to the LS202 form is controlled by the product type defined for an individual user.
- The product type defined for an employer or group only affects access to the LS202 form when new users profiles are created. If a high level user drills down into a lower level and creates a new user profile, the new user will inherit the product type defined for the employer or group.
- LS202 data is maintained separate from other types of data (OahsWC and Acord) with 2 exceptions. Information summary data presented on the Home page combines LS202 data with other data types as indicated by the footnotes on the Home page. OSHA related reports (OSHA, DART, Privacy, Sharps)include OshaWC data and LS202 data.
- The only summary reporting of LS202 data is via Reports >> Summary>> LS202 off of the main menu.
- If the FROLID data field is included in a LS202 Summary report, the FROLID will include an asterisk (i.e. 1205001*) to indicate that the LS202 incident has a related workers comp incident created. LS202 FROLIDs that do not include an asterisk have no related workers comp incident.
- Once an LS202 incident has been created, a related Workers Compensation incident record can be created in the same jurisdiction (i.e. Location State) by clicking on the WComp button at the bottom of the form. Note that "OSHA only" users will be taken to the OSHA 301 incident form and not the state form.
- LS202 incident related activities (i.e. Create, Edit, GoTo) start from the LS202 incident form. There is no way to perform these activities on a related workers comp incident record without going through the LS202 form first and then clicking the WComp button at the bottom of the form.
- All additional activities (i.e. Notes, CPA, Cost Tracking, etc.) are available for LS202 incidents and are initiated from the incident selection form, the LS202 form or the related Workers Comp form. Note that the system maintains one set of additional data records per LS202 incident regardless of where the activity is initiated. For example, opening the Cost Tracking form from the related Workers Comp form will display the same data as if the cost tracking form had been opened from the incident selection form or the LS202 form.
- LS202 Incidents can only be transmitted, hidden or deleted from the LS202 form. These buttons do not appear on the related Workers Comp incident form.
- The Report button on the LS202 form generates an LS202 report. On a related Workers Comp incident form the Report button generates the appropriate state report.
- LS202 incidents can be moved just like any other incident. If a related Workers Comp incident exists, it will be moved along with the LS202 incident.
This application is "Incident Record" based.
This page includes all the fields of information required to complete the OSHA log reports, OSHA related reports and "first reports of injury", FROIs.
If you used the “Form Assistant” many of the data fields are already complete. Verify that the pre-filled data is correct. Enter appropriate information in any empty fields.
All fields on the form except "User Defined Fields" are provided by State or Federal regulations. You are responsible for the completeness and accuracy of the information you enter and report.
It is not necessary to complete every field to generate a report. Fields that you leave blank can be completed later. Entries can be corrected or embellished by you later. Fields bordered in red are required in order to save the record in FROL (FirstReport Online).
Only the latest saved version of the data is stored, not revision histories. Generated reports are not stored. They are deleted when you exit the login session.

Navigation
The incident record data is all entered on one web page.
- There are no "Previous" or "Next" buttons needed.
- Attempts to "back" up may cause loss of data.
- "+" buttons display groups of data fields by topic.
- "-" hides the data fields by group.
- The "Security Log-out" 30 minute timer requires you to save your work to refresh the timer.
- If "Logged Out" by the timer on this form you may login again to access the form without losing your work.
- Re-enter your UserID and Password
- Select "Save" at the bottom of the form
- There is always only one data field per row
- Context "Help" is displayed in 3 rows below the Incidents >> Reports>> Manage>> commands.
- Click the cursor in the data entry field to have the "Help" text presented.
- The opening "O" and or "W" text shows if the field is required by OSHA, WC or both.
Data validation
There is no validation of the accuracy of your data input. There is validation of the format of the data type such as date, time or number of characters.
The required fields in red boxes are for the software system not regulation compliance.
Jurisdiction
The dark gray bar at the top of the form displays the jurisdiction that the form supports. Here are the types.

Verification data
The first section of fields in light gray (Employer Name...) cannot be edited. They are there for you to verify that your data is being entered in the correct area of the database.
The FROL ID # is also displayed. It is..

- Select one of the following descriptions to create a user-defined type of injury:
- All other specific injuries
- NOC (not otherwise classified)
- All other occupational disease/injury, NOC
- All other cumulative injuries, NOC
- Choose a previously defined custom description, or select “New” and enter your description in the blank field.
