Cause & Prevention Analysis (CPA) is a root cause analysis tool that enables loss control professionals to use available data to determine the root cause of incidents and suggest preventive measures.
Available as an optional feature, CPA is designed for risk engineers/consultants that enables them to add value to client relationships by using available client data to determine the root cause of incidents and suggest preventive measures. A true loss control function, CPA goes beyond forms and compliance to provide an outline for consultative services that can be performed by a risk engineer.

Process
The user starts with data already in the FROL system, answers a few more questions and makes checkbox selections from lists of pre-defined unsafe acts, unsafe conditions, direct causes, indirect causes and management controls. The user may also write a short narrative description of the selections that were made.
The CPA form my be partially completed and saved. All fields can be edited and saved any time in the future. The system only saves current data. No revision histories are save in the database.
Determine Cause
- The user
- starts with data already in the FROL system

- User defined items can easily be added to the lists if an appropriate pre-defined selection is not available.
- The user will be prompted to write a short narrative description of the selections that were made.
Suggest Preventive Measures
- Once the direct/indirect causes and management controls have been identified, the user is prompted to
- make checkbox selections of pre-defined preventive measures
- enter a narrative description of the selections made.
Preventive Accountability
- The user is prompted to assign target completion dates and individual responsibility for implementation of preventive measures that are selected.
Permissions
Only users who have specific authorization will be able to access the CPA function. The button that provides access to the CPA function will be invisible to users who are not authorized.
Types:
- None - will not see Analyze button or CPA record and summary reports
- Read only - all data
- Read only incident records, read/write CPA record and summary reports
- Read/write all
Getting permissions:
- MasterUserID - On
- Admin and Standard User -
- Create UserID if not already existing
- Submit UserID ONLY via email to support@firstreportonline.com requesting CPA activation
- You must be the primary contact to submit a request
- Usually takes less than one business day
- Same process to remove permission
Data input groups
Will hold all the CPA record data except CPA Case Notes. This CPA record is linked to this incident record exclusively.
- Incident Summary Read-Only fields
- Report Number
- Reflects the incident record's report number
- User edits permitted only in the incident report form
- The OSHA log requires it to be "unique"
- FROL ID Number
- System generated
- No edits permitted
- Absolute reference in the future if the report number is changed
- Other fields
- Reflect basic incident description data fields
- No reference to Employee, only report and FROL ID numbers

- Report Title will be printed at the top of the report. 30 Characters.

- Resets Security Log-out Timer to 30 minutes
- Returns to top of this form
- If User is "Read-Only" the page will refresh without saving any edits
- Report History
- An extended list of reports generated in this session prior to the "Last Report Generated"
- Click to display report in new window
- Right click mouse on link to "Save Target As" the file locally without opening
- Reports will be deleted at the end of this login session
