Zurich Incident Transmittal Form
No incident records are automatically "Transmitted" to a third party. The user must actively transmit the data.
The Incident Transmittal Form:
- Is only visible to Users with permission
- Displays key data to verify correctness before transmitting
- Builds a transmittal package Cover Sheet with contact information
- Permits generating copies of transmittal packages without transmitting to the recipient
- Creates a date/time stamp of the last transmittal for recordkeeping
- Confirmation will come directly from the recipient, not FROL
- Can be configured for unique recipients by Location Profile on request
Permission
If you don't see the "Begin Transmit Process" button at the top of an incident report form then your UserID has not been given that permission. There are three types of permissions.
- None
- Universal - all users automatically get this feature (default)
- By UserID - the account administrator provides an email request to FROL for a UserID to have this feature.
Data Fields
Completing the fields will build a transmittal package Cover Sheet with contact information to assist the recipient
The "Incident Transmittal Details" section of the form includes information that the Care Center requires for processing that may not have already been captured.
The completeness and accuracy of this section will enable the Care Center to process the incident more quickly and efficiently.
- All of the "Red box" fields require an entry.
- Contact Name, email, and Phone are prefilled if the User Profile contans those values. If you change these field values you will be asked to confirm them before proceeding.
- Some value must be entered even if it is N/A or something similar.
- If one or more fields are left blank, an error message will appear when later attempting to transmit a record.
- The "Customer Number" was entered in the Setup Form Employer Profile once and is automatically entered on this form.
- The "Contact" fields refer to the person actually transmitting the record in case a Care Center person needs the follow up on the "transmittal process" for that record.
Process
The "Begin Transmit Process" button is located near the top of the incident form. The button appears after the record has been saved at least once. Be sure to "Save" any new edits that you have made during this session before transmitting.
- Incidents should only be "Transmitted" once. Any changes to the report should be done for your own recordkeeping and for OSHA reports. Transmitting FROIs to Zurich will initiate the claims management process provided directly with Zurich.
- No data updates come back from Zurich into your FROL account electronically.
Incident Transmittal Form
After selecting the "Begin Transmit Process" button you will see an "Incident Transmittal Form". This form builds a "Cover page" of information that is needed by the Care Center to begin processing your claim.
Incident Overview - Information displayed in the "Incident Overview" has been captured from the FROL incident data record.
- This information is not editable in the "Incident Transmit Form".
- If you see an error or a blank field, return to the incident record form and change it there before proceeding.
Incident Transmittal History - The "Incident Transmittal History" will display if, or when, the incident report has been previously transmitted.
Incident Transmittal Instructions - "Incident Transmittal Instructions" are displayed for quick review.
Incident Transmittal Details - The "Incident Transmittal Details" section of the form includes information that the Care Center requires for processing.

Buttons
Selecting " Cancel " at the bottom of the page is the same as selecting the "Home" button in the task menu. The entries on this form will be discarded.
If Drop-list selection default is "Transmit (create data pkg and send)"
Select " Next " to generate a data package and send (transmit) the incident record to Zurich Care Center.
- Only Transmit once.
- The page will then refresh to the top of the "Incident Report Form".
- Note that below "Last Report Generated", an exact copy of the transmitted file is available as a link in the red bar for you to view and archive.
- Please save it immediately to a folder on your computer if you want to keep a copy.
- This report file will be deleted when you exit this session.
If you selected in the Drop-list "Report (create data pkg but do not send)"
Select " Next " to generate or re-generating this report without transmitting it.
- The page will then refresh to the top of the "Incident Report Form".
- Note that below "Last Report Generated", an exact copy of the transmitted file is available as a link in the red bar for you to view and archive.
- Please save it immediately to a folder on your computer if you want to keep a copy.
- This file will be deleted when you exit this session.
Transmittal Report
The "User Summary Page" ("Home") will now display in the "Information Summary" section the total of "Incidents Transmitted in the Last 7 days" and also "Incidents Transmitted Today". If there have been no transmittals in either period this row will not be displayed.
Notice that if there are records to report, the word description is a "Link".
- Select the link and a detailed report is presented in a new window.
- This HTML file can be viewed or saved and opened in MS Excel if you need to work with the data.
Step by Step
Transmitting FROIs to the Zurich Care Center

Open the incident form for the record to be transmitted

Note the red text under the button Begin Transmit Process

Review the data

If errors or omissions return to the incident form to correct

Zurich only wants FROIs transmitted once. Verify the history.

Select Next to send the FROI to the Zurich Care Center.

Select Report from drop list - Then Next to generate a report for your files without transmitting to Zurich.

Select Last Report Generated file to save or view the transmittal package. Note date stamp.
